Agentifying Patient Dynamics within LLMs through Interacting with Clinical World Model

Sepsis management in the ICU requires sequential treatment decisions under rapidly evolving patient physiology. Although large language models (LLMs) encode broad clinical knowledge and can reason over guidelines, they are not inherently grounded in action-conditioned patient dynamics. We introduce SepsisAgent, a world model-augmented LLM agent for sepsis treatment recommendation. SepsisAgent uses a learned Clinical World Model to simulate patient responses under candidate fluid--vasopressor interventions, and follows a propose--simulate--refine workflow before committing to a prescription. We first show that world-model access alone yields inconsistent LLM decision performance, motivating agent-specific training. We then train SepsisAgent through a three-stage curriculum: patient-dynamics supervised fine-tuning, propose--simulate--refine behavior cloning, and world-model-based agentic reinforcement learning. On MIMIC-IV sepsis trajectories, SepsisAgent outperforms all traditional RL and LLM-based baselines in off-policy value while achieving the best safety profile under guideline adherence and unsafe-action metrics. Further analysis shows that repeated interaction with the Clinical World Model enables the agent to learn regularities in patient evolution, which remain useful even when simulator access is removed.

Towards Video Anomaly Detection from Event Streams: A Baseline and Benchmark Datasets

Event-based vision, characterized by low redundancy, focus on dynamic motion, and inherent privacy-preserving properties, naturally fits the demands of video anomaly detection (VAD). However, the absence of dedicated event-stream anomaly detection datasets and effective modeling strategies has significantly hindered progress in this field. In this work, we take the first major step toward establishing event-based VAD as a unified research direction. We first construct multiple event-stream based benchmarks for video anomaly detection, featuring synchronized event and RGB recordings. Leveraging the unique properties of events, we then propose an EVent-centric spatiotemporal Video Anomaly Detection framework, namely EWAD, with three key innovations: an event density aware dynamic sampling strategy to select temporally informative segments; a density-modulated temporal modeling approach that captures contextual relations from sparse event streams; and an RGB-to-event knowledge distillation mechanism to enhance event-based representations under weak supervision. Extensive experiments on three benchmarks demonstrate that our EWAD achieves significant improvements over existing approaches, highlighting the potential and effectiveness of event-driven modeling for video anomaly detection. RelayGR: Scaling Long-Sequence Generative Recommendation via Cross-Stage Relay-Race Inference

Real-time recommender systems execute multi-stage cascades (retrieval, pre-processing, fine-grained ranking) under strict tail-latency SLOs, leaving only tens of milliseconds for ranking. Generative recommendation (GR) models can improve quality by consuming long user-behavior sequences, but in production their online sequence length is tightly capped by the ranking-stage P99 budget. We observe that the majority of GR tokens encode user behaviors that are independent of the item candidates, suggesting an opportunity to pre-infer a user-behavior prefix once and reuse it during ranking rather than recomputing it on the critical path. Realizing this idea at industrial scale is non-trivial: the prefix cache must survive across multiple pipeline stages before the final ranking instance is determined, the user population implies cache footprints far beyond a single device, and indiscriminate pre-inference would overload shared resources under high QPS. We present RelayGR, a production system that enables in-HBM relay-race inference for GR. RelayGR selectively pre-infers long-term user prefixes, keeps their KV caches resident in HBM over the request lifecycle, and ensures the subsequent ranking can consume them without remote fetches. RelayGR combines three techniques: 1) a sequence-aware trigger that admits only at-risk requests under a bounded cache footprint and pre-inference load, 2) an affinity-aware router that co-locates cache production and consumption by routing both the auxiliary pre-infer signal and the ranking request to the same instance, and 3) a memory-aware expander that uses server-local DRAM to capture short-term cross-request reuse while avoiding redundant reloads. We implement RelayGR on Huawei Ascend NPUs and evaluate it with real queries. Under a fixed P99 SLO, RelayGR supports up to 1.5$\times$ longer sequences and improves SLO-compliant throughput by up to 3.6$\times$.

P/D-Device: Disaggregated Large Language Model between Cloud and Devices

Serving disaggregated large language models has been widely adopted in industrial practice for enhanced performance. However, too many tokens generated in decoding phase, i.e., occupying the resources for a long time, essentially hamper the cloud from achieving a higher throughput. Meanwhile, due to limited on-device resources, the time to first token (TTFT), i.e., the latency of prefill phase, increases dramatically with the growth on prompt length. In order to concur with such a bottleneck on resources, i.e., long occupation in cloud and limited on-device computing capacity, we propose to separate large language model between cloud and devices. That is, the cloud helps a portion of the content for each device, only in its prefill phase. Specifically, after receiving the first token from the cloud, decoupling with its own prefill, the device responds to the user immediately for a lower TTFT. Then, the following tokens from cloud are presented via a speed controller for smoothed TPOT (the time per output token), until the device catches up with the progress. On-device prefill is then amortized using received tokens while the resource usage in cloud is controlled. Moreover, during cloud prefill, the prompt can be refined, using those intermediate data already generated, to further speed up on-device inference. We implement such a scheme P/D-Device, and confirm its superiority over other alternatives. We further propose an algorithm to decide the best settings. Real-trace experiments show that TTFT decreases at least 60%, maximum TPOT is about tens of milliseconds, and cloud throughput increases by up to 15x.

Right Side Up? Disentangling Orientation Understanding in MLLMs with Fine-grained Multi-axis Perception Tasks

Object orientation understanding represents a fundamental challenge in visual perception critical for applications like robotic manipulation and augmented reality. Current vision-language benchmarks fail to isolate this capability, often conflating it with positional relationships and general scene understanding. We introduce DORI (Discriminative Orientation Reasoning Intelligence), a comprehensive benchmark establishing object orientation perception as a primary evaluation target. DORI assesses four dimensions of orientation comprehension: frontal alignment, rotational transformations, relative directional relationships, and canonical orientation understanding. Through carefully curated tasks from 11 datasets spanning 67 object categories across synthetic and real-world scenarios, DORI provides insights on how multi-modal systems understand object orientations. Our evaluation of 15 state-of-the-art vision-language models reveals critical limitations: even the best models achieve only 54.2% accuracy on coarse tasks and 33.0% on granular orientation judgments, with performance deteriorating for tasks requiring reference frame shifts or compound rotations. These findings demonstrate the need for dedicated orientation representation mechanisms, as models show systematic inability to perform precise angular estimations, track orientation changes across viewpoints, and understand compound rotations - suggesting limitations in their internal 3D spatial representations. As the first diagnostic framework specifically designed for orientation awareness in multimodal systems, DORI offers implications for improving robotic control, 3D scene reconstruction, and human-AI interaction in physical environments. Step-Audio: Unified Understanding and Generation in Intelligent Speech Interaction

Real-time speech interaction, serving as a fundamental interface for human-machine collaboration, holds immense potential. However, current open-source models face limitations such as high costs in voice data collection, weakness in dynamic control, and limited intelligence. To address these challenges, this paper introduces Step-Audio, the first production-ready open-source solution. Key contributions include: 1) a 130B-parameter unified speech-text multi-modal model that achieves unified understanding and generation, with the Step-Audio-Chat version open-sourced; 2) a generative speech data engine that establishes an affordable voice cloning framework and produces the open-sourced lightweight Step-Audio-TTS-3B model through distillation; 3) an instruction-driven fine control system enabling dynamic adjustments across dialects, emotions, singing, and RAP; 4) an enhanced cognitive architecture augmented with tool calling and role-playing abilities to manage complex tasks effectively. Based on our new StepEval-Audio-360 evaluation benchmark, Step-Audio achieves state-of-the-art performance in human evaluations, especially in terms of instruction following. On open-source benchmarks like LLaMA Question, shows 9.3% average performance improvement, demonstrating our commitment to advancing the development of open-source multi-modal language technologies. DeepSeek-V3 Technical Report

We present DeepSeek-V3, a strong Mixture-of-Experts (MoE) language model with 671B total parameters with 37B activated for each token. To achieve efficient inference and cost-effective training, DeepSeek-V3 adopts Multi-head Latent Attention (MLA) and DeepSeekMoE architectures, which were thoroughly validated in DeepSeek-V2. Furthermore, DeepSeek-V3 pioneers an auxiliary-loss-free strategy for load balancing and sets a multi-token prediction training objective for stronger performance. We pre-train DeepSeek-V3 on 14.8 trillion diverse and high-quality tokens, followed by Supervised Fine-Tuning and Reinforcement Learning stages to fully harness its capabilities. Comprehensive evaluations reveal that DeepSeek-V3 outperforms other open-source models and achieves performance comparable to leading closed-source models. Despite its excellent performance, DeepSeek-V3 requires only 2.788M H800 GPU hours for its full training. In addition, its training process is remarkably stable. Throughout the entire training process, we did not experience any irrecoverable loss spikes or perform any rollbacks. Deep Learning for Video Anomaly Detection: A Review

Video anomaly detection (VAD) aims to discover behaviors or events deviating from the normality in videos. As a long-standing task in the field of computer vision, VAD has witnessed much good progress. In the era of deep learning, with the explosion of architectures of continuously growing capability and capacity, a great variety of deep learning based methods are constantly emerging for the VAD task, greatly improving the generalization ability of detection algorithms and broadening the application scenarios. Therefore, such a multitude of methods and a large body of literature make a comprehensive survey a pressing necessity. In this paper, we present an extensive and comprehensive research review, covering the spectrum of five different categories, namely, semi-supervised, weakly supervised, fully supervised, unsupervised and open-set supervised VAD, and we also delve into the latest VAD works based on pre-trained large models, remedying the limitations of past reviews in terms of only focusing on semi-supervised VAD and small model based methods. For the VAD task with different levels of supervision, we construct a well-organized taxonomy, profoundly discuss the characteristics of different types of methods, and show their performance comparisons. In addition, this review involves the public datasets, open-source codes, and evaluation metrics covering all the aforementioned VAD tasks. Finally, we provide several important research directions for the VAD community.
